Requirements for School Attendance
Rockford Lutheran School 2024-2025
The following items are required of all students attending school in the State of Illinois:
HEALTH EXAMS – A student must present proof of having a physical exam (within the 365 days) prior to entering:
- Preschool (PK)
- Kindergarten (K)
- 6th grade
- 9th grade
- any student entering from out-of-state, regardless of grade level
The State of Illinois “Certificate of Child Health Examination” form labeled “11/15” must be used. These forms are available at your doctor’s office.
The required health exam, immunizations and lead screening may be obtained at the doctor’s office of your choice (chiropractic not accepted).
New students who register after the first day of school have 30 days following registration to comply with the health examination and immunization requirements.
Proof of IMMUNIZATION and required boosters against a number of communicable diseases is required per the State of Illinois (refer to the Illinois Department of Public Health Website for exact specifications).
Students entering PK and K must show proof of LEAD SCREENING. It is mandatory for students living in ZIP codes 61101, 61102, 61103, and 61104.
State law requires that students without the required health exam, immunizations, and lead screening are to be EXCLUDED from school attendance unless certain exceptions are met by October 15, 2019. There are only two valid exemptions in Illinois to these requirements: medical or religious (both require signed documents from your health care provider).
DENTAL EXAMS – All Illinois children in K, 2nd, and 6th grades are required to have an oral health exam by a licensed dentist prior to May 15th of the school year. If a student fails to present proof of exam, RLS may hold the student’s report card until written proof of exam is received.
School dental exams must have been performed within the 18 months prior to the May 15th, 2020 deadline.
EYE EXAMS – All children enrolling in kindergarten and any student enrolling for the first time in a public, private, or parochial school (regardless of grade level) are required to have an eye exam no later than October 15, 2019. If a student fails to present proof of exam, RLS may hold the student’s report card until written proof of exam is received.
The eye exam must be completed by an optometrist or ophthalmologist.
Eye exams must be performed within the one year (365 days) prior to the first day of the school year.
